Qualifications

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• RN - Registered Nurse license required (state licensure and/or compact licensure).

Responsibilities

• Evaluates member needs and barriers to access.
• Develops and updates care plans with providers and resources.
• Coordinates between member/family and care team for patient-centered care.
• Provides follow-up, monitors status and revises plans as needed.
• Offers education to members and families about disease processes and benefits.
• Ensures compliance with state, federal, and payer regulators.
