The role is remote from anywhere in the United States, requires an active Michigan RN license to start, and follows a Monday–Friday 8am–5pm Eastern schedule, with emphasis on care coordination, barriers to access, and timely provider

Qualifications

• Requires a Nursing degree and an active RN license.
• 2–4 years of related experience in care management.
• Remote-friendly role with US-wide location options.

Responsibilities

• Develops, assesses, and facilitates complex care management activities for physical needs.
• Evaluates member needs, barriers to care, and coordinates with providers and resources.
• Develops care/service plans and revises them to meet goals.
• Ensures timely access to care and documentation for regulators.
• Provides education to members and families on disease processes and benefits.
• Maintains ongoing follow-up and tracks progress toward goals.
• Collaborates with leadership to improve quality and cost-effectiveness.
