Job summary

Providence is hiring a Care Manager RN for a remote, full-time role. The position follows 8-hour day shifts and focuses on coordinating care for complex patients across the continuum, while communicating with hospital staff and outside resources to ensure optimal resource use and outcomes.

The role requires an active California RN license, a nursing degree, and experience in acute care and care management or utilization review.

Qualifications

• Associate’s degree in nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in Medical-Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review, or completion of TIPS/Case Management Orientation.

Responsibilities

• Plan, coordinate, negotiate, procure and manage care for complex patients and families across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
• Monitor medical records for variances to clinical/financial standards and intervene to optimize outcomes.
• Incorporate risk management and infection control knowledge to assist the healthcare team.
• Identify liability or financial risk and collaborate with management and staff to minimize risk.
