Job summary

Nascentia Health is seeking a Care Manager to provide care management for health plan members, creating PCSPs, coordinating services, and documenting care per standards under supervision. This is a remote role with Monday–Friday, 8:00 AM–4:30 PM.

The position requires NYS RN license with BSN preferred and at least one year of nursing experience. Related care management certifications and bilingual skills are a plus. Sign-on bonus up to $15,000 and strong benefits are offered.

Qualifications

• Nursing license and NYS registration; BSN preferred; minimum one year nursing experience.
• Long-term care or managed care experience a plus.
• Knowledge of homecare, Medicare, and Medicaid is a plus.
• Care Management Certification is a plus.
• Bilingual skills a plus.
• Ability to work independently and prioritize workload.
• Computer literate with MS Office.

Responsibilities

• Provides care management for members through an interdisciplinary approach including assessment, planning, facilitation, and advocacy.
• Implements a quality, cost-effective plan of care in collaboration with member, family, and care team.
• Oversees caseload of managed care members.
• Coordinates health plan support staff and provides guidance to team.
• Participates in departmental meetings and required trainings.
• Adheres to HIPAA and organizational privacy policies.
• Communicates with potential members, families, and providers.
• Acts as liaison and member advocate.
• Demonstrates cultural competence and sensitivity to beliefs and languages.
• Other duties as assigned.
