Qualifications

• At least 2 years in health care, care management preferred.
• Registered Nurse (RN) with active license in state of practice.
• Valid driver's license and reliable transportation for travel as required.

Responsibilities

• Perform telephonic case management and transition-of-care tasks.
• Assess member needs, develop care coordination plans, and monitor progress.
• Collaborate with multidisciplinary team to ensure quality and cost-effective care.
• Conduct assessments within regulatory timelines and manage ongoing caseload.
• Assist with medication reconciliation and care plan adjustments as needed.
