Job summary

Molina Healthcare is seeking a Registered Nurse to support care management and LTSS activities. This remote role requires coordinating with a multidisciplinary team and visiting members as needed across La Crosse, Monroe, Trempealeau Counties.

You will assess needs, develop care plans, monitor progress, and facilitate waiver enrollment, with 25-40% travel. A valid RN license and strong communication are required. Molina offers competitive benefits and equal opportunity employment.

Qualifications

• Active RN license in-state.
• 2+ years in health care including care management or LTSS.
• Experience with disability or chronic conditions helpful.

Responsibilities

• Complete comprehensive member assessments within timelines including home visits.
• Develop and implement care plans with members and care team.
• Monitor and adjust care plans based on progress and needs.
• Coordinate LTSS and waiver services and enrollment.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Lead or participate in interdisciplinary care team meetings.
• Educate and motivate members using motivational interviewing.
