Job summary

UnitedHealthcare is seeking a Care Manager to support an Integrated Care Model, coordinating member needs and advocating for optimal care across services.

The role offers flexible 4x10 scheduling (7 am–6 pm) with no on-call or weekend requirements, and remote work options from Fayetteville, NC. The position includes competitive compensation with hourly pay between $29.00 and $52.00 plus benefits.

Qualifications

• Current, unrestricted RN licensure.
• 2+ years of clinical experience.
• 1+ years of MS Office experience.
• Reside in Mecklenburg, NC or surrounding areas.
• Access to a designated quiet workspace with PHI security.
• Reliable transportation and up to 25% travel within territory.

Responsibilities

• Assess, plan and implement care management interventions individualized for each member.
• Identify and initiate referrals for healthcare and community services.
• Develop and implement care plan interventions across the continuum of care.
• Communicate health-related information to stakeholders for coordinated care.
• Advocate for members to ensure needs and choices are represented by the care team.
