This role offers 4x10-hour days with no on-call or weekend requirements for eligible staff, plus remote work flexibility if located in Fayetteville, NC area.

The position emphasizes care coordination, collaboration with health teams, and ensuring access to necessary services, with a strong focus on member-centric care and outcome improvement.

Qualifications

• Current, unrestricted RN licensure.
• 2+ years of clinical experience.
• 1+ years of experience with MS Office (Word, Excel, and Outlook).

Responsibilities

• Assess, plan and implement care management interventions individualized for each member.
• Identify and initiate referrals for healthcare and community-based services.
• Develop and implement care plan interventions throughout the continuum of care as a single point of contact.
• Communicate required health-related information to ensure quality coordinated care.
• Advocate for members and families to represent and support their needs.
