Job summary

UnitedHealthcare is seeking an experienced Care Manager (RN) to support an Integrated Care Model. You will assess needs, plan interventions, and advocate for optimal services, ensuring members obtain appropriate care.

Role offers flexibility with remote work in Fayetteville, NC area and a 4x10 schedule option, plus no on-call or weekend requirements. Salary ranges hourly and benefits are provided based on eligibility.

Qualifications

• Current, unrestricted RN licensure.
• 2+ years of clinical experience.
• 1+ years of MS Office (Word, Excel, Outlook).
• Residence in Mecklenburg, NC or surrounding areas.
• Access to a dedicated quiet home workspace with PHI security.
• Ability to travel up to 25% within assigned territory.

Responsibilities

• Assess, plan and implement care management interventions tailored to each member.
• Identify and initiate referrals for healthcare and community-based services.
• Develop and implement care plan interventions as a single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for members and families to ensure needs are met by the care team.
