Job summary

UnitedHealthcare is seeking an RN Care Manager to support members remotely from Fayetteville, NC. This role focuses on assessment, care planning, and advocacy to ensure appropriate, quality care while coordinating with providers across the continuum.

You will enjoy a flexible 4x10 schedule (7am-6pm) and remote work options, with no on-call or weekend requirements after competency. The role requires NC RN licensure and 2+ years of clinical experience.

Qualifications

• Current, unrestricted RN licensure.
• 2+ years of clinical experience.
• Proficiency with MS Office (Word, Excel, Outlook).
• Reside in Mecklenburg, NC or surrounding areas.
• Access to a designated quiet home workspace with PHI security.
• Willingness to travel up to 25% within the assigned territory.

Responsibilities

• Assess, plan and implement care management interventions tailored to each member.
• Identify and refer to healthcare and community-based services as needed.
• Develop and implement care plans across the continuum as a single point of contact.
• Communicate required health information to ensure coordinated care expeditiously.
• Advocate for members and families to ensure needs and choices are represented.
