Job summary

Centene Corporation seeks an RN to develop, assess, and facilitate complex care management for members, including personalized care plans and family education. This remote role requires up to 75% local travel for in-home visits in the assigned territory.

Requires an accredited nursing degree or BSN and 2–4 years of related experience, plus active NJ RN licensure. Schedule is Mon-Fri, 8-5 ET, with flexibility.

Qualifications

• RN license in state or compact licensure required.
• Degree from an Accredited School of Nursing or a Bachelor's in Nursing.
• 2–4 years of related experience.

Responsibilities

• Develops, assesses, and facilitates complex care management activities for physical needs members.
• Conducts in-home member visits across the territory.
• Educates members and families to support high quality, cost-effective care.
