Qualifications

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2–4 years of related experience.
• RN - Registered Nurse licensure required (State Licensure and/or Compact State Licensure).

Responsibilities

• Evaluates member needs and barriers to care; facilitates care plans.
• Develops ongoing care/service plans and coordinates with providers and resources.
• Provides follow-up and monitoring of member status and progress toward goals.
• Ensures compliant documentation of all care management activities.
