Responsibilities

• Evaluate member needs and barriers to care, and facilitate the care plan.
• Develop ongoing care/service plans and collaborate with providers.
• Coordinate between member/family and care team for person-centered care.
• Provide follow-up and monitor status, revise plans as needed.
