You will assess needs, create care plans, and conduct coaching for caregivers, aiming to reduce burnout while maintaining compliance and quality care across territories.

Qualifications

• Active RN license required.
• Minimum two years' direct elder/adult care experience.
• Ability to travel within territory; valid driver’s license and insurance.

Responsibilities

• Conduct home visits to assess safety and health changes.
• Provide coaching and support to caregivers via app, refer resources and reinforce best practices.
• Develop trusting relationships to reduce caregiver burnout.
• Document and review records ensuring accuracy and regulatory compliance.
• Other projects and duties as assigned.
• Perform other duties as assigned.
