Careforth, Inc. is recruiting an RN Care Manager or Licensed Social Worker (LSW/LISW) in Ohio to onboard families and provide ongoing caregiver support. This role coordinates assessments, eligibility, and activation of services while coaching caregivers for daily health needs.

The position requires active Ohio licensure, two years of relevant clinical experience, and the ability to travel within the assigned area. A patient-centered plan of care and proficiency with digital tools are essential.

Qualifications

• Active Registered Nurse License in Ohio or Licensed Social Worker (LSW/LISW).
• Minimum two years of clinical care experience with elders or adults with disabilities.
• Ability and flexibility to travel within geographic area; valid driver's license and car insurance.

Responsabilités

• Onboard new families and assess eligibility for service.
• Guide Medicaid, waiver programs, and other benefits applications.
• Conduct initial clinical assessments and coordinate service activation.
• Provide caregiver onboarding to Careforth's HIPAA-secure platform.
