The position is remote (US-based) with a full-time schedule and pay ranging from $23 to $51 per hour. You will perform comprehensive assessments, develop and implement care plans, conduct visits as needed,

Qualifications

• Registered Nurse (RN) license must be active and unrestricted in state of practice.
• At least 2 years experience in healthcare, preferably in care management or a medical/behavioral health setting.
• Valid and unrestricted driver's license; familiarity with EMR and HIPAA regulations.

Responsibilities

• Complete comprehensive assessments of members per regulated timelines.
• Develop and implement care coordination plans in collaboration with members and healthcare professionals.
• Conduct telephonic, face-to-face, or home visits as required.
• Perform ongoing monitoring of care plans to evaluate effectiveness and suggest changes.
• Maintain an ongoing member caseload for regular outreach and management.
• Promote integration of services for members, including behavioral health and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and support members.
• Assess for barriers to care and provide coordination assistance.
• May conduct medication reconciliation as needed.
