Qualifications

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and identifies care management candidates.
• Develops and implements care coordination plans with member, family, and providers.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plans and documents interventions and outcomes, suggesting changes as needed.
• Maintains ongoing member caseload and proactive outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and collaborations with the care team.
• Uses motivational interviewing and Molina clinical guidelines to educate and motivate change.
• Assesses barriers to care and provides coordination and assistance.
