Molina Healthcare is hiring a remote RN Care Manager to support Medicare members through telephonic case management and transition-of-care activities. The role operates on Pacific Standard Time hours with a preference for candidates in Washington state.

Responsibilities include comprehensive assessments, care coordination plans, and ongoing monitoring. Travel may be required (25–40%), and collaboration with the multidisciplinary team is essential.

Competenze

• Must be a Registered Nurse with an active license in the state of practice (Washington preferred).
• 2+ years of healthcare experience, ideally in care management or related settings.
• Understanding of EMR systems and HIPAA.
• Knowledge of community resources and ability to coordinate services.
• Demonstrated time-management and prioritization skills.
• Strong verbal and written communication abilities.
• Proficiency with Microsoft Office and online portals.

Mansioni

• Complete comprehensive assessments within timelines to determine eligibility for care management.
• Develop and implement integrated care coordination plans with the member and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and document progress, adjust plans as needed.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of services across behavioral health and LTSS.
• Facilitate interdisciplinary team meetings and collaboration.
