Job summary

Molina Healthcare is seeking a Care Manager to support care coordination for LTSS in Wisconsin with remote work and travel to Chippewa Falls and Eau Claire counties.

You will conduct member assessments, develop comprehensive care plans, coordinate services, and monitor progress with a multidisciplinary team. Strong communication and documentation skills are essential, along with the ability to travel as needed.

Qualifications

• 2+ years health care experience including LTSS experience
• Experience with care management or medical/behavioral health setting
• Valid driver's license and reliable auto transportation for travel requirements

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required
• Facilitate waiver enrollment and disenrollment processes
• Develop and implement care plans with members and care network
• Monitor care plan effectiveness and adjust interventions as needed
• Promote integration of services including behavioral health and LTSS
• Assess medical necessity and authorize waiver services
• Collaborate with care managers and leadership as needed
• Travel 25-40% to member visits within Wisconsin
