Job summary

Careforth, Inc. seeks an RN Care Manager to provide personalized care to caregivers and loved ones. No nights or weekends; assess needs, create care plans, conduct home visits, and ensure excellent consumer care. Travel within Rhode Island is required and a flexible remote-first culture supports work-life balance.

Must have an active RN license and at least two years in elder or disability care, with proficiency in MS Office and case management systems. Competitive pay and benefits are offered.

Qualifications

• Two years of direct care experience for elders or adults with disabilities.
• Active RN license in the state of employment.
• Ability to travel within the assigned geographic area with valid driver’s license.

Responsibilities

• Conduct home visits to assess safety and health changes and coach caregivers.
• Follow care plans, provide ongoing coaching via app, refer resources, reinforce best practices.
• Develop trusting caregiver relationships to reduce burnout.
• Document records for accuracy and regulatory compliance.
• Take on other projects and duties as assigned.
