Qualifications

• RN license in OR/WA/ID in good standing.
• ADN or BSN required; BSN preferred for this role.
• Minimum of 3 years of ambulatory care or care coordination experience.
• Experience with discharge planning and multidisciplinary teams.
• Ability to review clinical documentation and provide patient education.

Responsibilities

• Coordinate care across the patient journey to reduce avoidable ED visits.
• Perform medication and allergy reconciliation per clinical protocols.
• Provide patient and caregiver education based on condition-specific knowledge.
• Communicate updates to PCPs and ensure timely follow-up.
• Support system-wide quality initiatives and care coordination programs.
• Maintain compliant documentation in the EHR and organize medical records.
