Namely is seeking California Based Nurse Care Managers for our Care and Case Management team. The role requires a California RN license, expertise in managing complex healthcare scenarios, and a passion for patient care.

Nurse Care Managers will deliver virtual care, create care plans, and work closely with a diverse clinical team to ensure optimal member health outcomes. Strong communication skills and empathy are essential, as well as the ability to work remotely.

Qualifications

• 2+ years experience working in care management.
• Willingness to become licensed in multiple states.
• Comfortable discussing medical conditions across age ranges.
• Strong competence with multiple computer/medical record systems.

Responsibilities

• Deliver coordinated, patient-centered virtual care management.
• Create impactful care plans with members and care team.
• Help members navigate complex medical conditions and benefits.
• Provide compassionate, longitudinal follow-up care.
