Job summary

Praxis Health is seeking a Nurse Care Manager for a remote role available to residents of Oregon, Washington, and Idaho. The position focuses on coordinating care, ensuring timely follow-up, and promoting evidence-based practice to improve outcomes and reduce hospitalizations.

The Nurse Care Manager will advocate for patients, educate on care plans, and collaborate with interdisciplinary teams to maintain continuity of care across settings.

Qualifications

• Associate Degree in Nursing (ADN) or BSN is required.
• At least three years of clinical experience in ambulatory care, care coordination, transitional care or discharge planning preferred.
• Proficient in EHR documentation and multidisciplinary team collaboration.
• Current RN license in Oregon, Washington, and/or Idaho; willingness to obtain Praxis-state licensure.

Responsibilities

• Serve as a patient advocate and navigator to guide patients and caregivers through the healthcare system.
• Review provider notes and discharge summaries to educate patients based on condition-specific knowledge.
• Conduct follow-up calls post-ER/hospital to reduce avoidable ED visits and improve access to care.
• Perform medication and allergy reconciliation per clinical protocols; verify names, dosages, and frequencies.
• Coordinate continuity of care across settings and communicate updates to PCPs.
