Job summary

Centene Corporation is seeking an experienced RN Care Coordinator to assess, plan, and implement complex care management for Oklahoma members. This field-based role requires an active RN license and strong communication, assessment, and care coordination skills to support members and families.

You will develop person-centered care plans, collaborate with providers, monitor progress, and educate on benefits and services, with a Monday–Friday schedule in a field/office setting.

Qualifications

• Requires RN license and active state licensure.
• 4–6 years of related experience in care management or equivalent.
• Experience with children, foster care, or juvenile justice is a plus.

Responsibilities

• Develops and continuously assesses care plans with providers to address unmet needs.
• Coordinates between member/family and care provider team for person-centered care.
• Monitors progress and revises care plans to meet goals and needs.
• Identifies barriers and intervenes for escalated cases in care management.
• Reviews member data and referrals to improve quality and compliance.
• Outreach via telephonic, digital, or home visits to assess needs and resources.
• Educates members and families on disease processes, care options, and benefits.
