Job summary

Centene Corp. is seeking an RN Care Manager in Oklahoma to assess, plan, and implement complex care management activities. You will develop personalized care plans, educate members and families, and coordinate with providers to ensure high-quality, cost-effective care.

The role requires an active RN license, a nursing degree, and 4–6 years of related experience. Work hours are Monday through Friday, 8:00 AM to 5:00 PM, with a pay range of $75,300 to $135,400 per year.

Qualifications

• Requires RN license and nursing degree; 4–6 years of related experience.
• Field-based care management experience in a health plan or Medicaid setting preferred.
• Strong communication, assessment, and care coordination skills.

Responsibilities

• Develops and continuously assesses care plans and collaborates with providers to address member needs.
• Coordinates between member/family and care teams to ensure person-centered care.
• Monitors status and revises care plans to meet goals and unmet needs.
• Reviews referrals and intake assessments to tailor service plans.
• Performs telephonic, digital, and in-home outreach to assess needs and resources.
• Coordinates with healthcare providers to adjust treatments and services as needed.
• Mentors new clinical hires and supports onboarding within the MCO.
