Job summary

Centene Corporation is seeking a registered nurse to develop, assess, and facilitate complex care management activities for members, coordinating with providers and families to deliver high-quality, cost-effective care.

This remote/hybrid role is based in Ohio with local travel for in-person member visits. Requires an RN license and 2–4 years of related experience; pay range $56,200–$101,000 annually, plus comprehensive benefits and flexible scheduling to support patient-centered care.

Qualifications

• RN or BSN degree from an accredited school.
• 2–4 years of related nursing experience.
• RN licensure in applicable state.
• Must be authorized to work in the U.S. without visa sponsorship now or in the future.

Responsibilities

• Evaluates member needs and barriers to care; develops or facilitates care plans.
• Identifies providers and community resources to address unmet needs.
• Coordinates between member/family and care team for person-centered care.
• Provides follow up and revises care plans to meet goals.
• Offers resource support for local services (housing, employment, etc.).
• Facilitates education to members and caregivers on disease processes and benefits.
• Ensures compliance with state, federal and payer regulations.
