Job summary

Centene is seeking a RN care manager to assess and plan complex care for members, coordinating with providers to ensure high-quality, cost-effective outcomes. The role involves travel to Wayne and Macomb counties up to 75%, with the balance of time spent working remotely from home.

You will develop personalized care plans, educate members and families, and collaborate with clinicians to optimize services, while ensuring compliance with state and federal regulations.

Qualifications

• RN with state licensure required.
• Bachelor's degree in Nursing or equivalent required.
• 4–6 years of related care management experience.

Responsibilities

• Develops and implements comprehensive care plans with providers to meet member needs.
• Coordinates between member, family, and care team to ensure person-centered care.
• Monitors progress and revises care plans to meet goals and unmet needs.
• Identifies barriers to care and initiates appropriate interventions.
• Reviews member data to improve quality and regulatory compliance.
