Job summary

Responsibilities

• Develops and implements complex care management plans with providers.
• Coordinates between member, family, and care team to ensure person-centered care.
• Monitors care plans and revises as needed to meet goals and needs.
• Educates members and families on disease processes, care options, and benefits.
• Collaborates with leadership to improve care delivery and quality.
• May precept new hires and support onboarding for care coordination.
