Job summary

Premise Health is seeking a Full-Time Registered Nurse, Care Manager to support client employees at our Health Center in Dalton, GA. The role emphasizes chronic disease management with a focus on bilingual member support (Spanish/English) and a largely remote execution (~90%), with occasional on-site events.

You will collaborate with a multidisciplinary team, develop care plans, educate members, and help improve health outcomes while anchoring privacy and compliance.

Qualifications

• Currently licensed RN in the state of practice.
• BLS or ACLS certification.
• BSN degree preferred.

Responsibilities

• Develops personalized treatment plans using provider plans and patient preferences.
• Supports a multidisciplinary team towards prevention, education and health promotion.
• Establishes supportive relationships with members.
• Assesses educational/clinical/social needs and prioritizes care.
