Job summary

A-Line Staffing Solutions is seeking a Registered Nurse for Chronic Care Management to proactively engage patients and manage large caseloads. The role supports medication management, physician escalations, and care-plan updates while using multiple EHR systems.

Ideal candidates have recent remote work experience, RN licensure, and comfort with unscripted clinical conversations. Work may be remote, on-site, or hybrid with EST hours.

Qualifications

• RN licensure required; current license in good standing.
• Experience with care management, HEDIS, or high-volume outreach preferred.
• Familiarity with multiple EHR systems and remote outreach workflows.

Responsibilities

• Make 50–60 outreach calls daily and manage 275–300 patients per month.
• Support medication refills, physician escalations, and care-plan updates.
• Educate patients through clinical conversations to improve outcomes.
