Job summary

Personify Health is seeking a registered nurse to coordinate wellness and disease management for members in a remote role serving the Pacific Time Zone. You will apply clinical knowledge in a managed care setting, review care gaps, create care plans, and liaise with primary care providers to optimize outcomes.

The position requires an active RN license with potential state licensure after hire, bilingual English/Spanish preferred, and willingness to travel.

Qualifications

• RN license required in United States.
• Additional state licensure may be required after hire.
• California, Washington and Oregon licenses required after hire.
• Experience in case management, wellness or disease management coordination.
• Clinical knowledge in managed care or population health.
• English/Spanish bilingual preferred.
• Willingness to travel.

Responsibilities

• Telephonically coordinate wellness and disease management for members with chronic conditions.
• Proactively contact targeted members to promote health and restore optimal functioning using care guidelines.
• Review gaps in care and data to develop individualized care plans with follow-up.
• Collaborate with members to assign primary care providers; facilitate referrals and high-cost medication reviews.
• Support Utilization Review processes and regulatory standards.
• Initiate referrals to case management, prenatal, wellness programs, and external services.
• Maintain complete documentation of case-managed members in designated systems.
• Track moderate/high-risk populations and associated interventions to improve health outcomes.
• Meet productivity, quality, and turnaround standards; participate in external audits.
• Maintain HIPAA compliance and confidentiality.
