Job summary

Premise Health in Mountain View, CA seeks a Full-Time Care Manager Registered Nurse to join our hybrid health center team. You'll work four days onsite and one remote day weekly, guiding clients through personalized care plans and coordinating with a multidisciplinary team.

The ideal candidate is a licensed RN with BLS, a BSN preferred, and at least three years of experience with chronic conditions. You will document care in Epic, support patient education, and help lower costs while improving

Qualifications

Licensed RN in state of practice.
Current BLS for healthcare providers.
BSN preferred.
At least three years’ RN experience caring for members with chronic conditions.
Certification in case management or diabetes education a plus.

Responsibilities

Develops personalized treatment plans using provider care plans and member preferences.
Supports a multidisciplinary team approach focused on prevention and health promotion.
Establishes supportive relationships with members and documents progress in EMR.
Collaborates with the clinical team to arrange further medical treatment when needed.
Coaches virtually and helps members take control of their health.
