Job summary

Honest Health is seeking a Nurse Care Manager to oversee patients with chronic conditions, collaborating with physicians and care teams. This fully remote role requires RN license in applicable states and focuses on reducing hospitalizations and optimizing outcomes.

You will build care plans, coordinate with hospitals, SNFs, home health, and vendors, monitor quality and cost, educate patients, and advocate for diverse social determinants of health.

Qualifications

• Active RN license in the applicable state(s) where role is based.
• BSN preferred; CCM credential preferred.
• 3+ years of clinical practice in hospital, clinic, home care, or nursing home.
• Experience with electronic health records or health registries required.
• Microsoft Office skills including Excel, Word, PowerPoint, Outlook.

Responsibilities

• Build empathetic relationships with patients and families.
• Develop individualized nursing care plans with patients and providers.
• Coordinate care across hospitals, SNFs, home health, and vendors.
• Monitor care outcomes and identify cost-effective measures.
• Attend huddles, interdisciplinary team sessions, and patient case conferences.
• Educate patients and advocate to empower self-management.
• Address social determinants of health such as transportation, housing, or food.
