Job summary

Point32Health in Massachusetts is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination, and disease management. The RN CM will work with members, caregivers, and providers to develop and manage member-specific care plans, with a focus on empowering wellness and self-management.

The role requires an RN license, a bachelor’s degree in nursing, and at least five years of clinical experience.

Qualifications

• Registered Nurse with current unrestricted license in state of residence; may require licensure in other states Point32Health operates.
• Bachelor’s degree in nursing or relevant equivalent experience.
• Minimum 5 years of relevant clinical experience; home care or case management preferred.

Responsibilities

• Perform telephonic and in-person outreach to enroll members in care management programs.
• Assess members to identify health goals and barriers to wellness.
• Develop and revise member-specific care plans in collaboration with the care team.
• Document care plans in appropriate systems per policy and regulatory standards.
• Provide health education and proactive strategies for chronic condition management.
• Coordinate safe transitions of care and ensure post-discharge instructions are understood.
• Collaborate with interdisciplinary care team to improve member outcomes.
• Participate in interdisciplinary rounds and commit to ongoing professional development.
