Qualifications

• Graduation from an accredited and approved nursing program.
• Active RN license in good standing required.
• CCM certification preferred (Certified Case Management).
• APM/CPC+, BPCI, MSSP experience preferred.
• 5–8 years of direct patient care experience in relevant settings.

Responsibilities

• Develop relationships with hospitals, case managers and discharge planners.
• Develop systems to engage high-risk members in case management.
• Ensure care coordination across home care, specialists and resources.
• Ensure post-discharge follow-up visits occur within 24 hours of discharge.
• Assist physicians and care team with preventive services, chronic care and disease management.
