Molina Healthcare is seeking a registered nurse for a 100% remote care management role on our fast-paced auto-dialer team. This position supports coordinated member care across the continuum to achieve desired outcomes and cost-effective care.

Preferred Illinois applicants. Responsibilities include assessments, care plan implementation, and collaboration with multidisciplinary teams. Travel up to 25–40% may be required depending on state need.

Qualifications

• RN license must be active and unrestricted in state of practice.
• 2+ years experience in health care, care management, or related setting.
• Knowledge of EMR and HIPAA.
• Travel up to 25-40% may be required based on state/contractual requirements.

Responsabilités

• Completes comprehensive assessments of members per regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plan with member, caregiver, physician and/or other health professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan to evaluate effectiveness and document interventions and outcomes.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services (behavioral health, LTSS, community resources) to improve continuity of care.
• Leads interdisciplinary care team meetings and collaborates with ICT.
• Uses motivational interviewing to educate, support and motivate change.
• Assesses barriers to care and provides assistance to address concerns.
• Provides consultation to peers as needed.
