Qualifications

• Active RN license in the state of practice.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines.
• Develops and implements care coordination plans with care team and network.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload with regular outreach.
• Facilitates integration of services across behavioral health, LTSS, and community resources.
• Participates in ICT meetings and collaborates with the care team.
• Assesses barriers to care and provides coordination assistance.
