Job summary

Molina Healthcare is seeking an RN for a 100% remote role on our auto-dialer team that is fast paced and high volume. Not your typical care management role.

You will provide support for care management and care coordination, collaborate with a multidisciplinary team to coordinate integrated member care, complete comprehensive assessments, develop and monitor care plans, and conduct visits as needed. 25-40% local travel may be required.

Qualifications

• RN license active in state of practice.
• Minimum 2 years in health care, care management or related setting.
• Valid driver license and reliable transportation for travel.
• Experience with EMR and HIPAA.
• Excellent communication and time-management skills.
• CCM certification preferred.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management.
• Develops and implements care coordination plan with member, caregiver, physician and other professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness and documents interventions and goals.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services for members, including behavioral health and LTSS.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change during member contacts.
• Assesses barriers to care and provides coordination and assistance.
