Molina Healthcare is seeking an RN to support care management and care coordination across the continuum in a 100% remote role. Collaboration with a multidisciplinary team and occasional in-person or telephonic visits are included, with 25–40% travel as required by state contracts.

The role emphasizes adherence to HIPAA, EMR proficiency, and implementing care plans to improve member outcomes. Active RN license and 2+ years of healthcare experience are required.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years healthcare experience, care management preferred.
• Valid driver's license and reliable transportation for travel.
• Familiarity with EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.

Responsibilities

• Provides support for care management/care coordination activities and collaborates with multidisciplinary team.
• Develops and implements care coordination plan with member, caregiver, physician and team.
• Performs telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Maintains ongoing member caseload and outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines to educate and motivate change.
• Assesses barriers to care and provides coordination assistance.
