Job summary

TAG MedStaffing is seeking a Registered Nurse Care Manager to support chronic disease management, patient education, and care coordination for a growing healthcare organization. This remote role requires Monday–Friday scheduling with remote work and occasional on-call rotation.

The ideal candidate holds an Associate or Bachelor’s in Nursing, an active RN license, and 2+ years in clinical care or care coordination.

Qualifications

• Associate or bachelor’s degree in Nursing from an accredited program is required.
• Active RN license is required.
• 2+ years of nursing experience in clinical care or chronic disease management.
• CCM or PCM experience preferred.
• Proficiency in EMR and digital healthcare platforms.

Responsibilities

• Conduct initial patient assessments and onboarding consultations to evaluate medical histories, symptoms, and needs.
• Develop and maintain individualized care plans addressing chronic conditions and interventions.
• Conduct regular patient check-ins to monitor health status and identify changes needing follow-up.
• Coordinate referrals, appointments, medication requests, and communications between patients and providers.
• Educate patients and families on chronic disease management and available resources.
• Collaborate with healthcare teams to maintain documentation and support care management initiatives.
