Job summary

Molina Healthcare is seeking an RN Care Manager to support AZ Medicaid members transitioning from hospital care. Role combines phone outreach and in-person visits with a focus on care coordination, behavioral health integration, and efficient documentation across systems.

Travel is required (local hospitals) with mileage reimbursement; schedule typically 7:00AM–6:00PM Arizona Time. Prior case management or inpatient experience preferred; remote work option available after six months for adjusted

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent experience.
• Registered Nurse (RN) license must be active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel within service area.
• Understanding of EMR and HIPAA requirements.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Provide care management/care coordination for Medicaid members transitioning from hospital to next care setting.
• Engage via phone and in-person visits; conduct assessments and develop care plans.
• Coordinate with multidisciplinary team and community resources to support member needs.
• Monitor and document care plan progress; adjust as needed.
• Travel 25-40% within local hospitals; mileage reimbursed.
