Job summary

Molina Healthcare is seeking six Texas-licensed RNs to support Medicaid members telephonically across the state. Positions require 2+ years in behavioral health or ICU/ED, with preference for prior managed care case management experience; bilingual is a plus.

Care managers will conduct assessments, develop care plans, and coordinate services with the care team. Telephonic work with occasional in-person visits, standard hours 8 AM–5 PM, in local time zone; travel up to 25–40% may be required.

Qualifications

• Must be a Texas-licensed RN with active license.
• Minimum 2 years in health care, care management or related setting.
• Valid driver’s license and reliable transportation.
• Experience with EMR/HIPAA.
• Bilingual is a plus.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plans with member and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plans, documents interventions and goals.
• Maintains ongoing member caseload and outreach.
• Facilitates integration of behavioral health and LTSS services.
• Supports ICT meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides assistance.
• May provide consultation to peers.
• Care managers may handle complex member cases and medications.
• May conduct medication reconciliation.
• Travel 25-40% may be required.
