Job summary

Molina Healthcare seeks six Texas-licensed RNs to work telephonically with Medicaid members across the state. Two tracks require 2+ years in behavioral health or addiction care; three require 2+ years in ICU/CCU/ED settings. Work mostly by phone with occasional in-person meetings; travel 25–40% as needed.

Hours are Mon–Fri 8am–5pm in your time zone; bilingual candidates are preferred. Ideal candidates have solid MS Office skills (Outlook, Excel, Teams, OneNote) and a background in care

Qualifications

• At least 2 years experience in health care, preferably in care management or related settings.
• RN license active and unrestricted in state of practice.
• Valid driver's license and reliable transportation for travel requirements.

Responsibilities

• Complete comprehensive assessments within timelines and determine eligibility for care management.
• Develop and implement coordinated care plans with members, families and providers.
• Conduct telephonic, in-person or home visits as needed.
• Monitor and document progress, adjusting care plan as necessary.
• Maintain ongoing member caseload with proactive outreach and coordination.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and interdisciplinary collaboration.
• Educate and motivate members using motivational interviewing.
• Assess barriers to care and provide resources to address concerns.
• May perform medication reconciliation and consult with peers.
