Job summary

Medica is seeking Care Coordinators (RN/SW) to support seniors and disabled members across Minnesota. This remote role involves telephonic or in‑home assessments, scheduling flexibility, and travel to member homes within Medica’s service area.

Qualifications

• RN diploma or degree (Associate or Bachelor) and a minimum of 2 years of community‑based service experience.
• Or a bachelor’s degree in nursing with a public health certification and a minimum of 1 year of community‑based service experience.
• 3–5+ years of experience beyond degree.
• Bachelor’s degree in social work.
• 3–5+ years of experience beyond degree, including 1–2+ years in community‑based services.
• Active, unrestricted RN or SW license in Minnesota.
• MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment.
• Public health certification preferred but not required.

Responsibilities

• Manage an assigned case load of members and create individualized care plans.
• Coordinate the delivery of approved medical and social services.
• Provide education surrounding Medicare and Medicaid benefits and serve as a primary contact for member questions.
• Partner with physicians, providers, and county financial entities to discuss care plans and update on member progress.
• Perform other duties as assigned.
