Job summary

Medica is seeking a Care Coordinator II or III (RN or Social Work) to support members across Minnesota. This remote role is home-based, with flexibility to choose a 4- or 5-day work week and a schedule free of evenings, weekends, and holidays.

You will coordinate individualized care plans, educate members on Medicare/Medicaid benefits, and partner with providers and county entities to ensure timely service delivery.

Qualifications

• Active, unrestricted RN License or SW License in MN.
• MnCHOICES Certified Assessor Training preferred or obtainable within 21 days.
• Public Health Certification preferred.
• Experience in home and community based services is advantageous.

Responsibilities

• Manage a caseload and create individualized care plans.
• Coordinate delivery of medical and social services.
• Educate members on benefits and address questions.
• Collaborate with physicians, providers, and county entities.
