Zusammenfassung

Medica is seeking a Remote Care Coordinator in Minnesota to manage a caseload of members, develop individualized care plans, and coordinate delivery of medical and social services. You will educate members about Medicare and Medicaid benefits and act as a primary contact to address questions and concerns.

Collaborating with physicians, providers, and county entities, you will discuss care plans, track member progress, and adapt plans as needed.

Qualifikationen

• RN Diploma or Degree with 2+ years in community-based services.
• Bachelor’s degree in Nursing with Public Health Certification + 1 year in community-based services.

Aufgaben

• Manage a specified caseload of members and create individualized care plans; coordinate delivery of medical and social services.
• Educate on Medicare and Medicaid benefits; serve as primary contact for members' questions and concerns.
• Collaborate with physicians, providers and county entities to discuss care plans and provide updates on member progress.
• Perform other duties as assigned.
