Job summary

Medica is seeking a Remote RN Care Coordinator to support Minnesota members with home-based assessments and telephonic care coordination. You will develop individualized care plans, coordinate services, and educate members on Medicare/Medicaid benefits.

The role offers a flexible 4- or 5-day work week, a schedule free of evenings, weekends, and holidays, and eligibility for mileage reimbursement as you travel to member homes.

Qualifications

• Active MN RN license required or ability to obtain.

Responsibilities

• Manage a caseload and coordinate delivery of medical and social services for members.
• Coordinate with physicians, providers, and county entities on care plans and updates.
• Educate members on benefits including Medicare and Medicaid.
• Travel within Medica's service area to meet members.
