Job summary

DispatchHealth is seeking a Virtual Nurse Coordinator (RN) to deliver hospital-level care in the home. This remote role requires strong clinical judgment and the ability to coordinate in-home and virtual care, serving as a patient advocate across care transitions.

The RN will collaborate with interdisciplinary teams, assess psychosocial needs, and leverage EHR and remote monitoring tools to ensure safety, quality, and timely interventions. Per diem schedule and travel to states may apply.

Qualifications

• Bachelor's degree in nursing is required.
• RN license in state of residence required.
• Current BLS/CPR certification required upon hire.

Responsibilities

• Assess and monitor patient condition using clinical judgment.
• Coordinate care across the interdisciplinary team and ensure seamless transitions.
• Serve as patient and family advocate with shared decision‑making and education.
• Collaborate with patients, caregivers, providers, and community resources to implement individualized care plans.
• Assess social determinants of health and environmental factors impacting outcomes.
• Utilize virtual care technologies, remote patient monitoring, and EHR systems for safe care delivery.
• Maintain accurate, timely clinical documentation per regulatory and organizational standards.
• Establish a collaborative relationship with patient and family to identify needs and access resources.
• Coach patients to reach optimum wellness and self‑management.
• Assess learning preferences and provide tailored education via appropriate modalities.
• Participate in daily interdisciplinary huddles and quality improvement activities.
