Job summary

Humana is seeking an Onsite Care Coordination Nurse in Oklahoma to support members through critical care moments with face-to-face assessments and lead care coordination efforts during admission and discharge.

The role emphasizes whole-person care, collaboration with providers and case managers, and thorough documentation while following safety and privacy standards. Working hours are 8:00–5:00 local, Monday–Friday.

Qualifications

• Current, unrestricted RN license in Oklahoma.
• At least 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.

Responsibilities

• Conduct face-to-face assessments of newly admitted members at acute care facilities.
• Educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care, safety concerns, and barriers to discharge; develop strategies to address these issues.
• Collaborate with attending providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures.
• Complete required documentation accurately and promptly.
• Maintain adherence to infection control, safety, and patient privacy standards.
