Job summary

Color is expanding its Virtual Cancer Clinic with a dedicated RN to support symptom triage, education, and care coordination across the United States via telehealth. You will work closely with physicians, genetic counselors, and care advocates to deliver guideline-based cancer screening and survivorship support.

The ideal candidate brings a BSN, an active RN license with NLC, and at least 7 years of nursing experience including primary care.

Qualifications

• BSN required and RN license active with NLC multi-state eligibility.
• Minimum 7 years nursing experience; at least 5 years in primary care with follow-up.
• Outpatient experience required; oncology experience a plus.
• Certifications such as AMB-C, CCM, CMGT-BC, OCN, AOCN, AOCNS required or to be obtained within 12 months of hire.
• Strong communication, empathy, and telehealth proficiency.
• Availability during Pacific Time business hours.

Responsibilities

• Triage, care coordination, and education via phone and async messaging.
• Conduct clinical and needs assessments.
• Telephone visits; coordinate follow-up and escalate as needed.
• Follow up on abnormal results; educate next steps and coordinate work-up.
• Review annual patient engagement surveys for evolving needs.
• Provide compassionate care and build trust with patients.
• Collaborate with Care Advocates to address non-clinical needs.
• Develop clinical protocols and best practices with Color Care Team.
• Engage with patients on symptom management, escalations, referrals, and in-person planning.
• Participate in care team sessions and document encounters thoroughly.
